If a U-boat is in a SubPen and it moves out and attacks in that same SZ and then survives any counterattack, it could then move back into the pen.

I had the same issue once and clearing the cache plus restarting the game fixed it. Also double check your firewall settings—mine was blocking something and caused the load to hang.
